A voter, or a voter's near relative may contact the Board of Elections Office to request an "Absentee Ballot Request Form". These forms are numbered and recorded with the voter registration number.

  • A "NEAR RELATIVE" is defined as a spouse, brother, sister, parent, grandparent, child, grandchild, mother-in-law, father-in-law, daughter-in-law, son-in-law, stepparent, stepchild, or verifiable legal guardian. A Power of Attorney is NOT acceptable unless the POA is also a near relative.
  • Each request must be signed by the voter or the voter's near relative. The near relative must also provide his/her name, address and relationship to the voter.
  • Absentee Ballots must be RECEIVED in the Board of Elections Office no later than 5:00 p.m. on the day before the election in order to be accepted.
